Gynecological Consultation

Gynecology is the specialty dedicated to the study, prevention, diagnosis, and treatment of diseases of the female reproductive system, i.e., the uterus, cervix, vagina, ovaries, etc., and mammary glands.

It is also related to and covers the specialty of obstetrics, which includes pregnancy monitoring or prenatal care with comprehensive care and attention to the mother and her needs, as well as monitoring her developing baby.

Gynecologists are responsible for the prevention, diagnosis, and treatment of diseases such as breast, ovarian, and uterine cancer; bartholinitis; hormonal changes; prolapse; amenorrhea; dysmenorrhea; menorrhagia; menopause; polycystic ovary syndrome; endometriosis; sexually transmitted diseases; and infertility.

In addition, they provide care to women during pregnancy, childbirth, or cesarean section, and postpartum, treating common infections due to common changes, counseling and placement on contraceptives or family planning methods, taking Pap smears and colposcopy for early detection of malignant cells or lesions, and, if necessary, performing biopsies of the vulva, vagina, cervix, or endometrium, as well as testing for sexually transmitted infections.

When should I go for a gynecological consultation?

It is extremely important to remember that visits should be made after you have begun your sexual life, with a routine gynecological checkup to prevent early detection and prevention of gynecological conditions.

However, there are certain symptoms or causes that we must learn to identify in order to see a gynecologist as soon as possible if necessary.

Below we list the most common ones:

 

1. Family planning: Get information, guidance, and advice on the wide variety of contraceptive methods available, so you can adopt the method that best suits your lifestyle, current needs, reproductive expectations, and health status.

 

2. Menstrual disorders: such as metrorrhagia, which refers to uterine bleeding not attributed to menstruation, regardless of its intensity; oligoamenorrhea, which refers to the absence of menstrual bleeding for more than 45 days and is not related to pregnancy or the use of any contraceptive method; menopause (the natural transition of the body to menopause, marking the end of a woman's reproductive years). All of this should be evaluated by a specialist.

 

3. Leucorrhea: This is excessive vaginal discharge that may be due to an infection or the presence of a foreign body in the female reproductive system. It is more common than disorders of red discharge.

4. Infertility: refers to the inability to conceive when a couple has had regular sexual activity for a year without contraception.

 

5. Genital prolapse: indicates the lowering of one of the pelvic organs. The uterus normally slides into the vagina. It affects women of any age, but is more common in postmenopausal women or those who have had one or more vaginal births.

 

6. Urination disorders: mainly due to incontinence, with sudden, altered changes in urination frequency.

7. Sexuality: We can attribute information, orientation, or disorder that makes sexual relations difficult or impossible, at any stage of the act.

 

8. Breast pathology: Any unusual change or symptom in the breast area; lumps, mastalgia (breast pain), changes in shape or texture, etc.

What does a routine check-up include and what is reviewed


When a woman goes to a gynecological consultation (unless it is for a specific problem) for the first time, the gynecologist proceeds with a routine examination. First, the consultation begins by taking the patient's vital signs, and a medical history is taken, in which certain points are addressed, such aso:


-Age and marital status: Identify if you have a steady partner.
-Sexual activity (active or inactive) and identify risk behaviors for STI acquisition.
-Obstetric formula (OC): full-term pregnancy(s) (more than 37 weeks)/preterm pregnancy(s), whether you had preterm twins, whether you adopted, whether it was an ectopic pregnancy, whether you had spontaneous abortions, or whether you had induced abortions. Live children. State delivery method (cesarean, forceps, vaginal).
-Contraceptive method (CM): Describe the contraceptive method you use.
-Last menstrual period (LMP): First day of your last menstrual period.
-Menstrual characterization: Number of days your period lasts and the interval between cycles.
-Pap smear (PAP): State the date of your last smear and the result of your Pap smear.
-Mammogram: Enter the date of the last mammogram and the radiological report (BI-RADS).
-History of surgery or related conditions: ovarian cyst, fibroid, adnexal lesion.

A breast examination is then performed to identify any lumps or abnormal discharge. A visual examination is also performed of the external part of the genital tract: the vulva, perineum, and introitus, or anterior part of the vagina. The specialist will use equipment such as speculums (vaginal mirror) or vaginal ultrasound (pelvic ultrasound) to examine the internal part of the genital tract: the vagina, uterus, and ovaries. A manual examination will also be performed, provided the patient has previously had sexual intercourse, wearing gloves at all times.

Sometimes, a colposcopy is performed, a type of close-focus binocular telescope that allows the doctor to view abnormal areas of the cervix in detail through the vagina.
